Please include child’s name, age, phone number including area code, city/state in addition to parent and/or guardian’s name. Upon selection the athlete will have the opportunity to be featured on the Football Hotbed website, social networking communication and receive a “live” phone interview on the Football Hotbed Talk Radio Show on Monday nights with your very own Mr. I.D. “The Mayor” & Mr. Brandon Odoi “ Mr. Sue Me For What?” .
